Zoom Video Conferencing Disclosure, Privacy Notice, and Recording Consent

This appointment or communication will be conducted Zoom Video Communications, Inc. (“Zoom”), a third-party video conferencing platform. By accessing or participating in the meeting, you acknowledge that Zoom operates independently and that your use of its platform is subject to Zoom’s Terms of Service and Privacy Policy. We encourage you to review those policies prior to participation.

1. Nature of the Communication

The meeting may involve the use of audio, video, chat, file sharing, screen sharing, and other interactive or collaborative features. These features may result in the collection of your voice, image, likeness, name, contact information, communications, shared content, technical data (such as IP address, device information), and other personal information you choose to provide or that is generated through your participation.

2. Recording Disclosure and Consent (All-Party Consent Compliance)

The meeting may be recorded, monitored, or otherwise captured in whole or in part, including audio, video, chat transcripts, and shared materials.

Where recording occurs:

• You will receive a clear and conspicuous notice at or before the commencement of any recording.
• Recording will not begin until such notice has been provided.

By affirmatively joining the meeting and continuing participation after receiving notice of recording, you:<p?

• Compliance, legal, and recordkeeping obligations

Information will not be used in a manner that is materially inconsistent with this disclosure or our Privacy Policy.

4. AI, Automated Processing, and Analytics Disclosure

To the extent applicable, meeting content (including recordings and transcripts) may be processed using automated systems, including artificial intelligence or machine learning tools, for purposes such as transcription, summarization, quality analysis, and operational improvement.

No automated processing is used to make solely automated decisions producing legal or similarly significant effects unless expressly disclosed separately.

• Service providers, vendors, and technology partners (including Zoom) who assist in delivering and supporting the meeting
• Affiliates and internal personnel on a need-to-know basis
• Legal or regulatory authorities where required by law or to protect rights and safety

All such sharing is subject to appropriate contractual, technical, and organizational safeguards.

Recordings and related data will be retained only for as long as reasonably necessary to fulfill the purposes described above, unless a longer retention period is required or permitted by law. Retention periods may vary based on legal, operational, or contractual requirements.

By joining and participating in the meeting, you acknowledge that you have read and understood this disclosure and consent to the collection, use, recording, and processing of your information as described herein and in our Privacy Policy, to the extent permitted by applicable law.

If you do not agree to these terms, do not join or promptly exit the meeting.
